We recognize that financial constraints can sometimes prevent individuals from accessing vital mental health support. Committed to transparency and ensuring equitable access to care, we have partnered with the College of Information & Technology at the University of Omaha to develop an intuitive tool that allows you to calculate your personalized estimated sliding fee for mental health evaluations. At For All Counseling, we firmly believe that prioritizing your mental well-being should be accessible to everyone, and cost should never be a barrier to getting the support and help you deserve.
Calculate your estimated mental health evaluation fee with our easy-to-use Sliding Fee Calculator. Enter your household size and annual household income below to see your estimated cost and learn more about how we make our mental health services accessible to a wider range of individuals and families.
To determine your eligibility and personalized fee for our mental health evaluations, we utilize a transparent and equitable sliding scale system. This system is designed to ensure that our services are affordable for individuals and families across a range of income levels.
Generally, eligibility for our sliding fee scale is based on your household size and your annual household income relative to the current Federal Poverty Guidelines for your household size. This ensures that those with the greatest financial need receive the most significant fee adjustments. Other factors, such as significant and unavoidable medical expenses or other extenuating financial circumstances, may also be considered. Please contact us if you believe your situation warrants additional consideration.
We first assess the total number of individuals residing in your household. This includes yourself, your spouse or partner, dependent children, and any other individuals you financially support and who you live with. We compare your household size to the current U.S. Federal Poverty Guidelines established by the Department of Health and Human Services (HHS) to establish your initial income bracket. These guidelines are updated annually and vary based on the number of people in a household.
We then consider your total gross annual household income from all sources before taxes. We then calculate an adjusted income benchmark by multiplying the relevant Federal Poverty Guideline for your household size by 225%. This adjustment factor accounts for the cost of living in our region and provides a more realistic income threshold.
Your actual total gross annual household income is divided by the adjusted income benchmark to determine a percentage that represents your relative income level compared to the adjusted poverty threshold. This percentage indicates the degree to which your income falls within or exceeds the adjusted poverty level.
For example, if your adjusted income benchmark is $50,000 and your actual annual income is $25,000, your percentage would be 50%.
This calculated percentage is then applied to our standard base evaluation fee of $1,000 to determine your personalized sliding fee. The lower your income percentage relative to the adjusted poverty guideline, the greater the reduction in your fee. The minimum fee in our sliding scale is intended to support client commitment and engagement in the process.
For instance, if your income percentage is 50%, your estimated fee might be $500. The specific percentages and corresponding fee reductions are outlined in our internal sliding fee scale, which is regularly reviewed to ensure fairness and accessibility.
This process ensures that individuals and families with lower incomes pay a proportionally smaller fee for our mental health evaluations, while those with higher incomes contribute a larger share, maintaining equitable access to our vital services for everyone in our community.
We understand that financial circumstances can vary significantly, and we are deeply committed to ensuring that high-quality mental health evaluations are accessible to everyone who needs them. In addition to our comprehensive Sliding Fee Scale, we are proud to offer a “Pay What You Can” model for individuals facing significant financial hardship.

The Sliding Fee Scale (SFS) program is a system we use to make our mental health evaluation fees more affordable. It adjusts the cost of our services based on your household size and annual household income. This means that individuals and families with lower incomes may pay a reduced fee compared to our standard rate. Our goal is to ensure that financial constraints do not prevent anyone from accessing the mental health support they need.
Using our Sliding Fee Calculator is simple and confidential. Just enter the total number of people in your household (including yourself and anyone you financially support who lives with you) in the “Household Size” field. Then, enter your total gross annual household income (before taxes) in the “Annual Household Income” field. Click the “Calculate Fee” button, and the tool will provide you with an estimated evaluation fee based on the information you provided and our sliding fee scale.
Eligibility for the Sliding Fee Scale is primarily determined by two factors: your household size and your total gross annual household income. We compare your household size to the current Federal Poverty Guidelines and then calculate an adjusted income benchmark. Your income is then compared to this benchmark to determine a percentage, which is used to calculate your adjusted fee. Our system is designed to be equitable, ensuring that those with greater financial need receive a larger reduction in fees.
Yes, the information you enter into our Sliding Fee Calculator is secure and confidential. The calculator is designed to provide you with an immediate estimate without storing your personal data. We are committed to protecting your privacy. When you contact us for further assistance or to discuss the “Pay What You Can” program, any information you share will be handled with the utmost confidentiality in accordance with our privacy policies.
We understand that even the estimated fee from the Sliding Fee Calculator may still be challenging for some individuals and families. If you find the estimated fee unaffordable, please do not hesitate to contact us. We encourage you to inquire about our “Pay What You Can” program. This program offers further financial flexibility for those experiencing significant hardship and may allow you to receive an evaluation for a reduced fee based on your ability to pay.
